Context
C-Score was a clinically validated health assessment app created in partnership between Chelsea Football Club and Huma, a leading digital health platform. Designed to help people understand and improve their general health through evidence-based assessments, the app launched publicly in June 2021 in partnership with Diabetes UK during Diabetes Awareness Week. Available on iOS and Android, it reached 700,000 people through social media alone, achieved a 51% click-to-install ratio, and was endorsed by Chelsea FC ambassador Carlo Cudicini.
My role
Senior Product Designer within the Chelsea Digital Ventures (CFC) product team, working directly with Huma's GPs, UX researchers and engineers. I designed the end-to-end app experience across all health domains. Primary user base was 50+, which shaped every decision around accessibility, clarity and clinical compliance.
What I worked on:
Assessment flows for each health domain: sleep, alcohol, smoking, resting heart rate, waist to height ratio, reaction time and self-rated health
Score results screens with contextual medical guidance and peer comparison data
Dashboard showing overall C-Score and domain completion progress
History tab with interactive graphs showing behaviour trends over time
Projection tool letting users simulate the impact of behaviour changes on their future score
Design system built to meet clinical compliance and accessibility standards throughout
